Information for new patients
What to expect at your first visit
We welcome new patients at every location. Your first appointment will include a review of your medical history, a physical exam, and a conversation about next steps. Our goal is to help you understand what’s causing your symptoms and what can be done to treat them.
Before you come in
- Please bring your photo ID, insurance card, and any forms you’ve filled out in advance.
- If you’re coming for a surgical consult or a wound evaluation, bring a list of your medications and any recent test results.
- If you’ve been referred by another provider, it’s helpful if we can review any notes or imaging ahead of time.
Insurance
We accept most major insurance plans, including Medicare. If you’re not sure whether your visit is covered, you can call our office or check with your insurance carrier directly. Some plans may require a referral.
Dowloadable Forms
You can print and complete these forms before your appointment to save time during check-in:
HIPAA Privacy Notice
Receipt of Privacy Practices
New Patient Information Form
